Registration Packets
Packets are available to download and print for either Football or Cheerleader Registration. All forms required for registration are included in the packet as well as the reference documents. (Please print single-sided only.)
What to Bring
- Copy of child’s birth certificate
- Copy of child’s health insurance card
- Recent photo (like a school photo)
- $100 registration fee per child: cash, check or venmo (@rjvikings) are accepted
Who Can Register
Children must be going into Kindergarten through 8th grade for NEXT school year 2025/2026 which begins in September to be eligible to participate.

If you download and print either of the Registration Packets, then you do not need any other forms or documents below. They are available however, in case you have an issue downloading the complete packet or need to access individual pages to download one at a time. All documents will be available to fill out at registration as well.
